Intralipids
New research has suggested that women who have experienced recurrent miscarriages or multiple failed IUI or IVF cycles as a result of natural killer cell activation may benefit from the use of intralipids. Intralipids are synthetic and made from 1.2% egg yolk phospholipids, 10% soybean oil, 2.25% glycerin and water. It is administered through an IV 4-7 days before embryo transfer or insemination.Natural killer cells are regulated by the immune system. In women who have autoimmune issues, the natural killer cells can react abnormally to an implanting embryo, treating it as an invading cell and signaling for the body to attack it. Studies have found that intralipids can help to deactivate the natural killer cells, allowing the embryo to implant on the uterine wall and grow normally. Intralipids are re-administered 4-5 weeks
following a positive pregnancy test, to keep the natural killer cells deactivated until the pregnancy can override the signals being sent by the immune system.

Intralipids (approximately $100 per administration) are far less expensive than Intravenous Immunoglobulin (IVIG) (approximately $2000 per administration), and initial studies show comparable efficacy for deactivation of natural killer cells and pregnancy rates. Intralipids are well tolerated by patients with few side effects and are created synthetically, unlike IVIG which is a blood product. It takes about an hour and a half to two hours for the intralipids to be administered through the IV.

Donor Embryos
albanyFor years patients have had the opportunity to create their families
using donor sperm and donor eggs through the services at CNY Fertility
Center. Now CNY Fertility
Center is proud to offer
embryo donation and adoption as a family building option.

 

Patients who undergo IVF treatments may be left with extra embryos
(fertilized eggs), which can be cryopreserved (frozen) for use during future
cycles. Once the patients have completed their family, they may still have
extra cryopreserved embryos remaining. This can place patients in a difficult
position when they begin to discuss what to do with the remaining embryos. The
previous options available have included: donate the remaining embryos to
research, keep them frozen, dispose of them or donate them to other patients
through an outside agency. CNY

now offers embryo donation and adoption through our center.

The goal of the Embryo Donation Program is to match these remaining embryos to
recipient parent(s) who are interested in the concept of adoption, but would
also like to carry the child and establish a special bond over the nine month
gestational period. The embryos are genetically related to the donor parents,
but all legal rights to the embryos and/or resulting child(ren) are relinquished
by the donor parents and given to the recipient parent(s).

 

Medical backgrounds of each biological donor parent are available
for the recipient parent(s). This allows the recipient parent(s) to choose the
embryos that would be most suitable for the family they wish to create.

a family attainable

to additional couples and individuals in our local
community, as well as internationally.

PICSI/HBA Multi-Site Clinical Trial
 
albanyIntracytoplasmic Sperm Injection (ICSI) is used for about 99% of patients at CNY Fertility Center who are doing an In Vitro Fertilization (IVF) cycle. It is the process in which an embryologist directly injects one sperm into a mature egg, in an attempt to fertilize the egg, creating an embryo.The embryologist selects the sperm using a microtool called an ICSI needle, under a large, high-tech microscope. The sperm are evaluated for their progression (swimming), as well as their morphology (shape), however neither of these attributes can definitively tell the embryologist that the sperm is mature.

A PICSI dish contains hyaluronan microdots, to which only mature sperm can bind. Once the sperm are added to the dish the embryologist is able to select only the bound (mature) sperm, using them for injection into the egg. The concept is that mature sperm are more likely to be chromosomally normal, potentially increasing the fertilization rate and subsequent pregnancy rates.

CNY Fertility Center Syracuse is participating in a multi-site clinical trial of the PICSI dish’s effectiveness. We are currently recruiting patients to be in the clinical trial. Fish Oil

Emerging studies and literature have recognized fish oil as
a potential fertility enhancing supplement. As a result, the benefits of fish
oil have been discussed in the news and fertility books, encouraging health
care providers to recommend it to patients.

 

Fish oil is full of Omega-3 Essential Fatty Acids (EFAs).
Omega-3 EFAs are important because researchers and clinicians are discovering they
help suppress natural killer cells, moderate rheumatoid arthritis, help reduce
endometriosis, decrease inflammation and increase uterine blood flow to prevent
clotting (which in some cases leads to miscarriage). Once pregnant, they help
by creating an environment in the body that is conducive to healthy brain and
nervous system development in the fetus.

 

Like many vitamins and supplements, all fish oil capsules
are not equal in quality or benefit. Lower grades of fish oil not only taste funny,
but may be created with parts of the fish that contain high levels of toxins.
Vegetable forms of Omega-3 EFAs are not digested as well, wasting the potential
benefit. Pharmaceutical grade fish oils such as the Usana brand that CNY Fertility
Center and CNY Healing
Arts offers to clients (Biomega 3) not only have a wonderful citrus aftertaste,
but are certified to be free of heavy metal toxins and are easily digested.

 

As an acupuncturist, I’m often prescribing fish oil to
clients on the path to parenthood. Women who experience painful periods, have
scar tissue, endometriosis, fibroids, cysts or autoimmune issues, are
recommended to take Biomega 3 in a dosage of 2000-4000 mg per day. I suggest
2000 mg per day for all women who are trying to conceive either naturally, or
with fertility treatments. Men with a varicocele or sperm morphology issues are
recommend to take 2000 mg per day along with herbs and Proflavanol 90, which is

Maya Abdominal Massage
Maya Abdominal Massage (MAM) is an external, non-invasive
massage technique used to align pelvic and abdominal organs – specifically the reproductive
organs. The massagetherapist repositions the organs, releasing physical and emotional
congestion, which encourages the free movement of blood and chi (energy). MAM
uses techniques developed through the study of the body and the relationships
of the joints, ligaments and muscles, in conjunction with herbology and ancient
Mayan healing traditions. Men and women benefit from MAM for treatment of

Week Nineteen: The Changing Seasons

I cannot believe I have been writing my online articles since early May! Thank you to those who have commented and encouraged me along the way. I have to say, I love reading your comments!!!

As autumn begins, I think we are all adjusting to more rigid schedules. Although I often mourn the loss of summer, I have to say I am thankful to have had such a spectacular summer! In my Week Nine article I discussed how summer is an excellent time to simply enjoy the moment, and I am happy to report I did just that. The dog and I took many walks, my husband and I enjoyed a late summer vacation in the Berkshires and I read a number of fiction and non-fiction pieces, some of which I shared in my weekly online articles.

Just as I set a summer intention to be more spontaneous and to live in the moment, I realize that I need to develop a new intention for this new season. As I ponder what my mind, body and spirit need most, I know I must remember that there will always be a pile of work waiting for me and my to-do list will never quite be complete. Whose is? I have to confess that in the midst of my summertime spontaneity, I neglected some of the more significant parts of my routine, primarily my Gratitude List and yoga. Read more

Hea12homeGood morning, and I hope you had a wonderful weekend!  It appears as though the season is quickly changing over, and bringing in some beautiful colors, smells, and tastes.

I will often receive emails, or questions about retaining one’s identity during the fertility process.  Sometimes we can feel as though we have lost a bit of ourselves in all of the appointments, medications, waiting, and stress.  It is common to hear ‘but I promise, I used to be so happy, and relaxed.’  It is very easy to feel as though the happy, and easygoing ‘you’ is gone.  However, I want to emphasize that the person you speak of has not been lost.  You are still you.  We cannot always play one role in life, our concerns and focus moves freely as we grow older and change various circumstances in our lives.  For example, you cannot be the same person you were in college as you are now while you are trying to conceive.  That fun-loving and carefree college student is not gone, but that role is not in the forefront of your mind and heart.

I can promise you that you will smile and laugh again.  You will evolve and change through this process, but you will not completely abandon who you are.  In The Voice of Knowledge Don Miguel Ruiz has a wonderful quote about accepting ourselves, and allowing ourselves to live as we feel – not how others tell us to.  You may encounter situations similar to this when you are feeling down because of some bad news such as a negative pregnancy test, and someone tell you to ‘cheer up’ or ‘let it go’. It is easy for them to say because they are not feeling exactly what you feel.  Try not to get too down on yourself for feeling these emotions of sadness and depression.  They are completely normal, and it is what your mind, body, and spirit need to feel.  If you find you are having difficulty moving past these emotions once you have fully experienced them, then I suggest you try the process I outlined for you last week.  However, remind yourself that what you are feeling is normal, and good because you are just how you are supposed to be.

Take a moment to reflect on Don Miguel’s words, and if you would like to further discuss any of this, I am always available.

The first step toward improving your relationship with yourself is to accept yourself just the way you are.  You don’t need to learn how to love yourself.  You need to unlearn all of the reasons why you reject yourself, and by nature you love yourself.  You love not the image you project or the way you are, but you love yourself because of what you are.  Then you start to enjoy yourself until you love yourself so much that you give yourself everything you need.  You don’t leave yourself until last anymore.  The more you enjoy the presence of yourself, the more you enjoy your life, and the more you enjoy the presence of everyone around you. (p. 169)

PICSI/HBA Multi-Site Clinical Trial

Week eighteen: Grand Opening

Not only does Dr. Kiltz know how to throw a Grand Opening party, he knows how to reach out to others. The minute he greets anyone he gives him or her a hug, and when a patient leaves his office, a hug usually accompanies any necessary paperwork as well. Additionally, he strives to meet his patients’ needs by offering daily words of encouragement and numerous workshops for couples dealing with infertility. He also strives to integrate Eastern and Western medical practices. Dr. Rob’s desire to help his patients achieve their dreams of creating a family is reflected in his truly caring demeanor. By reaching out to each patient, he takes the monotony out of the process and gives each patient a personal boost of encouragement.

Too often, we don’t make the time to reach out to others. What I truly appreciate about CNY is that someone there always takes the time to reach out to me, and at the Rochester Grand Opening on Tuesday, CNY reached out to over 100 people! Everyone from the employees of the medical office next door to current and past patients to town officials were in attendance. Kristen Magnacca, author of Love & InfertilityOnce again, Dr. Rob Kiltz has invited Kristen Magnacca to share the Love and Infertility Workshop Series at his CNY Fertility Centers and CNY Healing Arts Centers in NY. It begins with Part I ~  “I just want a baby! Change the questions that you’re asking yourself and succeed.” Are the questions that you’re asking yourself driving you crazy? Why can everyone else get pregnant but me? What will happen if I don’t … create enough follicles? Ovulate? What will happen if this cycle does not work?

The series will run a bit different from the last session, as she will be rotating each month between the three center locations:

Syracuse on Wednesday, September 30th, 2009, 6:00-8:30pm – there’s still time to register.
Albany on October 6th, 2009, Tuesday, 6:00-8:30pm
Rochester November 17th, 2009, Tuesday, 6:00-8:30pm

24-autumn-tree‘We will see you in two weeks for your blood test.’  This simple phrase is loaded with excitement, anticipation, and anxiety.  Two whole weeks until you are able to find out if the cycle was successful.  What do you do for two weeks?  How do you keep yourself occupied?  What about all of those anxious ‘what ifs’?   I receive a request for help regarding the two-week wait almost weekly.  This is an incredibly common challenge for our patients- and I remember that terrible, sinking feeling myself.

Recently I posted a response to the CNY Fertility Discussion Forum (found here: Discussion Forum) regarding how to cope with these moments of anxiety and questioning during this dreaded wait.  Although I cannot tell you exactly how to free yourself of these feelings, I can hopefully offer some advice and a process that worked for me.  Below you will find the process I used, and also posted.

In regards to getting through this final week, try not to let your worried thoughts become repetitive. This can sound challenging to do, but it is incredibly important. If we allow our mind to continually travel to negative thoughts and questions, it can cause a cycle of greater anxiety and depression. Although we cannot completely prevent negative or anxiety producing thoughts, we can actively work towards moving past them.
