CNY’s Fertility Clinic in Philadelphia is under the direction of Dr. Gennadiy Ivanov.  Dr. Ivanov is a Fellow of the American College of Obstetrics and Gynecology (FACOG), a member of the American Medical Association (AMA) and American Association of Gynecological Laparoscopists (AAGL).  He is board-certified in obstetrics and gynecology in the U.S. and Ukraine.  Dr. Ivanov completed his OB/GYN studies and medical residency in 1999 at Kharkov State Medical University in the Ukraine, where he subsequently practiced as a university faculty physician.  In 2009, he completed his U.S. medical residency at the University of Buffalo in New York.

Dr. Ivanov focuses on Gynecology and Women’s Preventive Health. He is passionate about using minimally invasive procedures in gynecology to avoid major surgeries. Dr. Ivanov joined CNY Fertility in 2020 and has received extensive training from some of the nation’s leading experts in fertility care. In addition to being the head physician at CNY’s Philadelphia clinic, he is an active staff member at St. Mary and Holy Redeemer Hospitals and fluent in English, Russian, and Ukrainian.

Fertility Treatments Available to Patients at our Philadelphia Fertility Clinic

IUI 

Unless otherwise advised, patients typically begin fertility treatment with the least aggressive treatment. Many consider intrauterine insemination (IUI and otherwise known as an artificial insemination), either with or without medications before moving on to in vitro fertilization (IVF). 

For women with health tubes and a normal hysterosalpingogram (HSG), IUI is a good starting point. Around the time of ovulation, specially washed sperm is placed past the cervix and directly into the uterus using a thin catheter. This puts the sperm closer to the egg, giving them a better chance of fertilizing the egg. Prior to the insemination, the patient is monitored via ultrasound and bloodwork to track hormone levels and egg development so that the insemination is properly timed. 

Compared to IVF, IUI is very affordable at just $650 per insemination. The cost of medications (if used) are not included and can vary considerably depending on your protocol.

CNY Fertility Philadelphia does not currently perform inseminations at the Philadelphia office. That said, all of the testing and monitoring appointments which make up the majority of the office visits during an IUI treatment are done at the Philadelphia fertility clinic. The inseminations themselves will be done at one of our Upstate offices in Syracuse, Albany, Buffalo, or Rochester New York. 

IVF In Philadelphia

Some clients choose IVF as their first treatment because it is the most successful fertility treatment available today. Others choose IVF after less aggressive treatment has been unsuccessful. During the IVF process, a fertility specialist removes one or more mature eggs from the follicles developing inside the woman’s ovaries, fertilizes the egg with partner or donor sperm, and grows the embryo(s) inside the embryology laboratory until it gets transferred back into the carrier’s uterus.  

Similar to IUI, IVF retrievals and transfers are done at one of CNY’s full-service locations, with all monitoring before egg retrieval and embryo transfer performed in Philadelphia.

Mini IVF

As an alternative to traditional IVF, IVF using reduced or minimal stimulation to recruit a few high-quality eggs, called Mini IVF, is also an option at CNY Fertility.  Mini IVF allows clients to save money on medications, effectively reducing their overall treatment cost.  It also helps decrease the risk of ovarian hyperstimulation and the odds of having a large number of remaining embryos after achieving a clinical pregnancy.

Tubal Reversals

Another travel treatment available through CNY Philadelphia is a tubal ligation reversal. This is the process by which a surgeon restores a woman’s natural fertility by repairing her fallopian tubes after undergoing a sterilizing tubal ligation surgery (commonly known as having one’s “tubes tied”).

Like traveling for IVF, the actual surgical procedure is performed at one of CNY Fertility’s New York locations, but pre-surgical assessments is completed in Philadelphia, which helps to limit travel. 

Tubal reversals are usually not covered by health insurance, Medicare, or Medicaid because they are typically considered elective surgeries. The self-pay price for a Tubal Reversal at CNY is $4,995 and covers anesthesia, the surgery itself, and a 2-week post-operative check-up. Not only that, but a discounted IVF cycle is available if you are unable to achieve pregnancy within a year (for those <35) or six months (if >35).

Genetic Testing 

Alongside IVF, many couples elect to genetically test their embryos to determine the healthiest embryo for transfer. Preimplantation Genetic Testing (PGT) gives parents critical information about their embryos’ health, allowing them to choose the sex of their child and deselect embryos with genetic abnormalities or other hereditary conditions. Genetic testing requires a Frozen Embryo Transfer (FET) because it take some time to get the test results back.

Donor Eggs at our Philadelphia Fertility Clinic

CNY offers a carefully screened selection of egg donors for clients who wish or need to use donor eggs.  Donor eggs give recipients similar odds of success as the young fertile women who donated the eggs and are an ideal option for couples who are carriers of a genetic condition, who’ve experienced repetitive IVF failure, have diminished ovarian reserve or poor egg quality, single men or male couples, and anyone else who needs healthy eggs to grow their family.

Vasectomy Reversals and Sperm Extractions

Men who’ve electively undergone a vasectomy as a permanent form of birth control have several options if they now wish to father a child. The first is a vasectomy reversal, where the two halves of the vas deference are reconnected.  This is the only option for couples who wish to conceive naturally through intercourse. If that’s not possible, a second option is sperm extraction, medically referred to as Percutaneous Epididymal Sperm Aspiration (PESA) or Testicular Sperm Aspiration (TESA).  Both are surgical procedures used to retrieve sperm from some part of the male reproductive tract when there is no sperm present in the semen. With sperm extraction, IVF is necessary.

Fertility Preservation at our Philadelphia Fertility Clinic

For people looking to preserve their fertility for whatever reason—medical treatment, more time to pursue other goals, or military service, Egg Freezing, Embryo Freezing, and Sperm Freezing are optional services. Sperm freezing is a relatively easy and quick process.  Sperm can be frozen the same day you contact CNY Fertility and can be stored indefinitely. A single sperm sample usually produces around five vials of sperm.  Most artificial insemination attempts require 1-3 vials for each attempt. For this reason, we recommend men store multiple samples.

The process of egg or embryo freezing is slightly more complicated and requires several stages:  stimulation, egg retrieval, and finally, Intracytoplasmic Sperm Injection (ICSI) if freezing embryos.  Embryo preservation offers a higher likelihood of future pregnancy versus egg freezing.

How it Works

After your consultation for travel IVF and receiving full instructions for your treatment you’ll begin taking your medications. At this point, you will need to start monitoring. The team in Philadelphia will check hormone levels in your blood and measure your follicles, egg, and uterine lining every few days until your trigger. The CNY team reviews your results, changes medications as necessary, and informs you of the next steps. Our travel team is available 24/7 to answer your questions or address concerns.

Your egg retrieval is usually scheduled midway through your stimulation cycle once your team has a chance to see how your follicles and eggs are developing. After your egg retrieval is scheduled, you can plan your travel to one of our New York offices. The number and duration of your trips are contingent on whether you’re having a Fresh Embryo Transfer or a Frozen Embryo Transfer (FET). Patients are asked to be in the area the day before any procedure and should not leave until the day after any procedure.
